You introduce the patient; we manage screening, patient communication, secure information gathering, pathway selection, coordination and follow-up. If our assessment is not the right answer, we facilitate a more appropriate route with a useful referral summary.

Referral process

A simple introduction. A managed pathway.

The practice does not need to decide the assessment type or transfer clinical records through ordinary email.

    Refer

    The patient scans the practice QR code or you send them the referral link.

  2. 02
    Secure intake

    We provide the patient with secure instructions for submitting forms and documents and manage the required coordination.

  3. 03
    Free screening

    The clinical question, evidence and intended decision are reviewed; screening is not a diagnosis.

  4. 04
    Right pathway

    We either facilitate a more appropriate referral or offer an assessment with clear price and timing.

  5. 05
    Report and follow-up

    With patient consent, relevant outcomes and recommendations are returned to the referrer.

The practice or referrer receives no financial benefit for a referral. A diagnosis, NDIS decision or other third-party outcome is not guaranteed.

A digital brochure and practice-specific QR code are prepared at no charge. If print is requested, quantity, delivery and any cost are confirmed separately before proceeding.
