Embark Potential is led by a Registered Psychologist and provides a guided pathway for children and adults with psychological, developmental or cognitive concerns. Your practice makes the introduction; we manage free screening, patient and family communication, secure information gathering, clarification of the assessment, report or service required, and coordination of the appropriate next step.

From practice setup to the appropriate next step.

The practice does not need to decide the assessment type or transfer clinical records through ordinary email.

    Request setup

    Provide the practice name and work contact details. No patient information is required.

  2. 02
    Receive link, QR and brochures

    Within 48 hours, we create your practice-specific referral link and patient QR code. We also design, print and send practice-specific patient brochures at no charge.

    Introduce the patient

    Give the patient the QR code or send them the referral link.

  4. 04
    Free screening

    We contact the patient or family, gather relevant information securely and clarify the assessment, report or service required.

    Appropriate next step

    Before any paid work, we explain the scope, fee, timeframe and outputs of a suitable Embark service—or manage the complete no-cost referral process to a more appropriate service.

After screening, we carry the pathway through to an actionable next step.

The no-cost pathway does not end with deciding against an Embark assessment. If another service is more appropriate, we manage the referral process.

If an Embark service is appropriate

An agreed service with clear clinical accountability

  • We confirm the scope, fee, timeframe and outputs before any paid work begins.
  • We manage documentation, information gathering, booking, coordination, assessment, reporting and agreed follow-up.
  • Every final diagnostic report is reviewed and signed by Arman Naeini, Registered Psychologist.
  • With explicit consent, we prepare and send summaries tailored to the role and needs of necessary recipients.

If another service is more appropriate

A no-cost referral—from service search to follow-up

  • We search for an appropriate external service.
  • With explicit consent, we prepare a recipient-specific clinical referral summary and collate relevant evidence and documents.
  • We send the referral directly where permitted; when a GP referral is required, we send the summary and relevant documents to the GP.
  • We follow up to confirm receipt and progress towards acceptance or another clear outcome.

The practice or referrer receives no financial benefit for a referral. A diagnosis, NDIS decision, funding or school decision, external-service acceptance or other third-party outcome is not guaranteed.

When you submit the form, limited technical and security information, such as your IP address, browser or device information and submission time, may also be recorded.

Providing this information is voluntary. Your phone number and note are optional. Without the other required information, we may be unable to process your request.

Information provided through this form is transferred directly to and stored in Embark Potential’s Replit production environment, configured for Replit’s Australia publishing geography. It is not routed through ordinary email. Replit is a United States-based provider and may use overseas personnel or subprocessors for infrastructure, security, operational or support functions, as explained in our Privacy Policy.
