When complex case consultation may help

A case may be complex because several problems interact, information is incomplete or contradictory, previous approaches have stalled, risk is changing, stakeholders disagree, or no one is holding the full clinical picture.

Consultation can support psychologists, behaviour support practitioners, clinical leads, service providers and multidisciplinary teams. Individuals and families may also enquire when psychological, behavioural or support needs have become difficult to navigate.

You do not need to arrive with a service label. Start with what has become difficult and what decision now needs to be made.

What Embark does

We organise the available information, distinguish evidence from assumption and identify the questions that remain open. We then help determine what further records, data, observation, consultation or assessment would materially improve the decision.

The work may examine the presenting difficulty, formulation of the case, practitioner reasoning, team coordination, implementation barriers and the service system surrounding the person. The aim is not simply to add another opinion, but to produce a clearer and defensible next step.

How the engagement works

  1. Clarify the decision
    We define what needs to be understood or decided and who will use the consultation.
  2. Agree scope and consent
    Roles, records, confidentiality, fees and relevant permissions are confirmed before substantive review.
  3. Review the evidence
    Relevant reports, history, data and stakeholder accounts are organised and critically examined.
  4. Identify uncertainty
    We state what is supported, what remains unclear and which missing information could change the conclusion.
  5. Develop and review the pathway
    A reasoned plan is produced and can be revised as new information or outcomes emerge.

Consultation is often mainly written so that questions, reasoning and decisions remain documented. Scheduled discussions can be added where they improve the work.

Possible outputs

Depending on scope, outputs may include a case formulation, evidence and uncertainty map, risk or decision analysis, information-gathering plan, end-to-end action pathway, written consultation record, stakeholder briefing, professional report or recommendations for further assessment and review.

The exact output is agreed before work begins. Embark will not create a report simply because a report has been requested if another form of work would answer the question more appropriately.

A defined piece of work

This is not an open-ended or automatically ongoing service. After a brief fit discussion, we define the decision to be answered, the information to be reviewed, the output, timeframe and fee. You receive a written quote before substantive work begins.

If the agreed work reveals a new question requiring a separate assessment, report or longer engagement, we explain that separately. Nothing additional begins without written agreement.

What this service is not

Complex case consultation is not emergency or crisis response, ongoing case management or a substitute for the responsibilities of a treating clinician, behaviour support practitioner, case manager, support coordinator or service provider. It is not therapy unless a separate psychological service has been explicitly assessed and agreed.

If the initial review indicates that Embark is not the right fit, we will say so and suggest a more appropriate next step where possible.
